To confirm your Fed Ed booking you will need to fill out the payment form. If you wish to pay by credit card, you will need to email or fax the details with the form to firstname.lastname@example.org / 03 9663 3652. If you would prefer to pay by cheque, please send it with your payment form to:

When the payment transaction has been completed this form can be used as a receipt for school tax purposes.
Cancellations must be received by email or fax at least one week prior to the scheduled booking. Cancellations incur a 10% administration fee. Any cancellations after this time will carry a 50% liability payment.
